To submit your child's immunization records, please provide the updated documentation to the school Health Attendant either in person or via email at [email protected].

Medication Orders

For medication order forms, ensure they are completed and signed by your child's healthcare provider and yourself, then deliver them along with the medication in its original, properly labeled container to the school's Health Attendant.
